New Member Application - Online Form

Please read this entire page before completing an application.  To receive a copy of the application to join Azionaqua, click on the link below to open the Adobe® Acrobat file and print the application.  You may then fill out the application and turn it in at the pool to swim the same day!*  Please note: we are ONLY able to accept checks at the pool.  You may also choose to pay by sending the application through the mail to:

If you choose to send the application through the mail, please be sure to have it signed and dated on the back by yourself AND a sponsor.  If we receive an application at the pool or through the mail without a signature or sponsor, it will be sent back.  Also be sure to read the entire application before you sign it as you will be held responsible for the agreement.  You are also required to obtain a copy of the pool rules and regulations from the front desk during your first visit.

As soon as we receive your application you will be mailed cards within a few weeks.  Admittance to the pool will be possible before you receive your cards by presenting a valid ID at the front desk.  We will then print a temporary pass for you to use until you receive your cards in the mail. 

*Partial payments are not accepted
*Payments will be accepted at the pool starting opening day (Saturday of Memorial Day weekend)
